단백동화 안드로겐성 스테로이드를 사용하는 근력 운동 선수에서 심근 초음파 반사도 측정을 통해 조기 심장 이상을 포착할 수 있다는 연구 결과가 나왔다.
국제학술지 《에코카디오그래피》2026년호에 발표된 연구에서 브라질 연구진은 141명의 근력 운동 선수를 스테로이드 현재 사용자와 비사용자로 구분해 심근 조직 특성을 비교했다.
ciB는 초음파가 심근에서 반사되는 강도를 심낭 반사도로 교정해 측정한 값이다. 수치가 높을수록 심근 섬유화 또는 조직 변성이 의심된다. 분석 결과 CU 그룹의 ciB는 평균 -19.54 dB로,NU 그룹보다 유의하게 높았다.
ciB 상승은 좌심실 질량 지수, 이완 기능 지표인 E/E',좌심방 경직도와 유의한 양의 상관관계를 보였다. 반면 수축 기능을 나타내는 좌심실 전체 종적 변형률과 박출률은 두 그룹 사이에 차이가 없었다.
이 결과는 AAS 사용에 따른 심장 이상이 수축 기능보다 이완 기능 단계에서 먼저 나타남을 보여준다. 연구진은 "ciB 측정이 일반 심초음파 검사로는 포착하기 어려운 초기 심근 조직 변화를 발견하는 지표로 활용될 수 있다"고 밝혔다.
이완 기능 이상은 초기에 증상 없이 진행하는 경우가 많아 간과되기 쉽다. 전문가들은 AAS를 사용하는 선수에 대해 정기 심장 검진 시 ciB를 포함한 조직 특성 평가를 함께 시행할 것을 권고하고 있다. 근력 운동 선수라면 스테로이드 사용 여부와 관계없이 정기 심장 초음파 검사를 받는 것이 바람직하며, AAS 사용자라면 이완 기능 이상 여부를 세밀하게 모니터링해야 한다.

Resistance-trained athletes who use anabolic-androgenic steroids (AAS) show detectable changes in myocardial tissue characteristics through calibrated integrated backscatter (ciB) echocardiography, according to a new study published in Echocardiography in 2026.
Researchers in Brazil enrolled 141 resistance-trained athletes — 120 men and 21 women — dividing them into current AAS users (CU, n=95) and non-users (NU, n=46). The team used ciB, an ultrasound-derived measure of myocardial acoustic reflectivity calibrated against the pericardium, to assess myocardial tissue properties in both groups.
The CU group demonstrated significantly higher ciB values than non-users (−19.54 dB vs −20.88 dB, p=0.047). Higher ciB values are associated with increased myocardial fibrosis or structural remodeling. In further analyses, elevated ciB correlated positively with left ventricular mass index (LVMI), the E/E' ratio — a marker of diastolic filling pressure — and left atrial stiffness.
Critically, systolic function parameters including global longitudinal strain (GLS) and ejection fraction (EF) did not differ between the two groups. This dissociation indicates that diastolic dysfunction precedes detectable systolic impairment in AAS users — consistent with prior research on steroid-associated cardiomyopathy.
The findings carry important implications for cardiac screening in strength athletes. Standard echocardiography may not reveal the early myocardial changes that accumulate with AAS use, particularly in athletes who remain asymptomatic. The ciB technique adds a layer of tissue-level assessment that could help sports medicine physicians detect subclinical pathology before it progresses to overt cardiac disease.
The authors noted that the absence of differences in systolic function does not mean the myocardium is unaffected. Rather, the data suggest an ongoing process of structural remodeling that has not yet compromised pump function but may represent an early warning sign. Left atrial stiffness, in particular, is increasingly recognized as a predictor of adverse cardiovascular outcomes.
Clinicians and sports medicine professionals working with strength athletes are advised to incorporate ciB and diastolic function assessments into cardiac surveillance protocols, especially for those with known or suspected AAS use. Athletes using AAS should undergo regular cardiac evaluations and discuss long-term cardiovascular risks with qualified physicians. Even those who have discontinued steroid use may benefit from follow-up imaging to track potential cardiac remodeling over time.
