전방십자인대 재건술과 반월판 수술을 함께 받은 환자가 수술 1~2년 후에도 착지·런지·피벗 동작에서 정상 무릎과 비교해 관절운동학적 이상이 지속된다는 연구 결과가 나왔다.
버몬트대학교 연구팀이 "Orthopaedic Journal of Sports Medicine"에 발표한 이번 연구는 ACL 재건술과 반월판 수술을 함께 받은 12명을 대상으로 이중 형광투시와 MRI 기반 연골 모델을 결합해 동적 3차원 관절 역학을 분석했다.
분석 결과, ACLR+M 수술 무릎은 착지와 런지 동작에서 굴곡각이 유의미하게 감소했다. 또한 착지와 피벗 동작에서 경골의 전방 이동이 증가했다. 연골 접촉 분석에서는 착지와 피벗 동작 시 경골 연골면의 접촉 위치가 후방으로 이동하고 내측·외측 구획 모두에서 접촉 중첩이 증가하는 패턴이 관찰됐다. 런지에서는 경골 접촉 경로의 전후방 이동 거리가 증가했다.
연구팀은 이러한 관절운동학적 이상이 외상 후 골관절염 발생 및 진행의 조기 생역학적 선행 요인이 될 수 있다고 분석했다. ACLR만 시행한 경우보다 반월판 수술을 동반할 때 PTOA 발생률이 더 높다는 선행 연구와 일치하는 결과다.
연구팀은 수술 후 1~2년 시점의 고강도 동작 시 비정상적 무릎 역학을 조기에 인지하고 이를 완화하기 위한 맞춤형 재활 및 모니터링 전략의 필요성을 강조했다.
Patients who undergo combined anterior cruciate ligament reconstruction and meniscal surgery (ACLR+M) continue to exhibit significant alterations in joint kinematics and articular cartilage arthrokinematics during demanding dynamic activities at one to two years after surgery, according to a new study published in the Orthopaedic Journal of Sports Medicine.
Researchers from the University of Vermont used dual fluoroscopy combined with MRI-based cartilage models to assess three-dimensional tibiofemoral mechanics in 12 participants (8 male, 4 female) who had undergone ACLR+M, analyzing three activities: single-leg landing, lunging, and pivoting.
Compared with the contralateral knee, the ACLR+M knee exhibited significantly reduced flexion during landing (p<0.001) and lunging (p=0.04), and increased anterior tibial translation during landing (p=0.01) and pivoting (p=0.002). Articular cartilage analysis revealed posterior shifts in contact location on the tibial cartilage surface and increased contact overlap in both medial and lateral compartments during landing and pivoting. During lunging, the ACLR+M knee showed increased anteroposterior excursion of the tibial contact path.
The authors argue that these arthrokinematic deviations represent early in vivo biomechanical abnormalities that may contribute to the elevated incidence of posttraumatic osteoarthritis (PTOA) observed after ACLR+M compared with ACLR alone.
The findings carry important clinical relevance, highlighting the need for targeted rehabilitation and monitoring strategies that address altered loading patterns during high-demand functional activities to mitigate long-term joint degeneration.
