Ankle injuries are the most common basketball-related setbacks — but a new decade-long epidemiological study reveals that the type of injury and how it happens varies significantly by sex and age, pointing to the need for more targeted, personalized prevention strategies.
Researchers at the University of Florida published a descriptive epidemiology study in the *Orthopaedic Journal of Sports Medicine*, analyzing data from the National Electronic Injury Surveillance System (NEISS) on 20,303 basketball players aged 13-35 years who visited emergency departments (EDs) between 2014 and 2023. Female players made up 15.3% of the cohort.
Key findings revealed distinct injury patterns by sex. Female players were 33% more likely than males to seek emergency care for ankle sprains and strains (OR = 1.33; 95% CI, 1.19-1.47), while male players had significantly higher odds of ankle fractures (OR = 0.57) and dislocations (OR = 0.46). Injury mechanisms also differed: male players were more frequently injured during poor landings (10.0% vs. 6.8%), while female players experienced more falls (10.8% vs. 8.5%), collisions, and contact-related incidents.
Age was also a significant factor. Players aged 19-24 and 25-35 had 31% and 22% greater odds of ED visits for sprains/strains, respectively, compared to the 13-18 age group, while adolescents had the highest rates of fall- and collision-related injuries. Across all groups, 98.4% of patients were treated and released, with only 0.4% requiring hospitalization.
As a descriptive epidemiology study using surveillance data, this research cannot establish causation and has limited information on injury severity or return-to-play outcomes.
Practical takeaway: Prevention strategies should be tailored by sex and age group. Youth players (13-18) benefit most from balance and neuromuscular coordination programs. Female players should prioritize ankle stability and proprioceptive training. Male players should focus on landing biomechanics and dorsiflexion mobility to reduce fracture and dislocation risk.
