Blood flow restriction (BFR) training offers an appealing way to stimulate muscle with light loads. However, increased muscle activation and actual functional performance improvement are distinct outcomes.
This pilot randomized controlled trial examined the effects of four weeks of low-load BFR training on quadriceps surface electromyography (sEMG) activity and functional transfer. Participants were assigned to a low-load BFR group or a control group, with pre- and post-intervention sEMG and functional testing.
The BFR group showed decreased quadriceps sEMG activity during task performance compared with controls, suggesting that less muscle activation was required for the same task. While this may indicate improved muscular efficiency, it also suggests limited functional transfer. No significant differences emerged between groups on functional tests including jumps and sprints.
These results indicate that BFR training alters muscle activation patterns but may have limited transfer to functional performance over short four-week periods. Longer durations or combination with high-load training may be necessary for meaningful performance gains.
BFR training remains useful during early rehabilitation or when high loads are contraindicated. However, expecting performance enhancement from BFR alone may be unrealistic; it is better utilized as a bridge toward high-load training. Cuff pressure is typically set at 50 to 80 percent of arterial occlusion pressure, with loads at 20 to 30 percent of one-repetition maximum.
