It starts as a nagging ache just below the kneecap. You rest, you do your eccentric exercises, you try to train through it -- but the pain keeps returning. For competitive athletes dealing with refractory patellar tendinopathy, the road back to full fitness can stretch into months or years. A new randomized study from Japan tests whether combining two evidence-based treatments -- platelet-rich plasma (PRP) and extracorporeal shock wave therapy (ESWT) -- into a single bio-mechano-therapy approach can accelerate the structural healing that monotherapy often fails to achieve.
Dr. Shimizu at Nippon Sports Science University randomized 10 competitive athletes with chronic, refractory patellar tendinopathy into three groups: PRP alone (n=5), ESWT alone (n=2), and combined PRP plus ESWT (n=3). All participants followed a standardized eccentric rehabilitation program in parallel. The study, published in Cureus in April 2026, assessed clinical outcomes using VAS pain scores, VISA-P, IKDC, Lysholm score, and KOOS. Structural changes were tracked by MRI and ultrasonography at baseline and at one, three, and six months.
All three treatment groups showed significant improvement over time in clinical outcome measures (p less than 0.01), with no statistically significant between-group differences in patient-reported scores. However, structural imaging told a more differentiated story. At the six-month mark, the PRP plus ESWT group showed significantly superior results in ultrasonographic hypoechoic lesion area (p=0.031) and intratendinous lesion length (p=0.015), with moderate-to-large effect sizes (epsilon-squared 0.34-0.37). All 10 athletes successfully returned to sport within six months, with no between-group differences in return-to-play timing.
The study's most significant limitation is its tiny sample size -- particularly the ESWT-only group with just two participants, making group comparisons statistically fragile. This is preliminary evidence, not a definitive answer. Still, the biological rationale is compelling: PRP delivers growth factors that promote cellular repair, while ESWT applies mechanical stimuli that activate mechanotransduction pathways. Combining both may target tendon healing from two complementary angles simultaneously.
For athletes stuck in the chronic injury cycle despite structured rehabilitation, this study adds to the case for exploring combination therapy with a sports medicine specialist. Alongside any injection-based treatment, progressive tendon loading -- particularly heavy slow resistance training -- should remain central to any rehabilitation plan. The structural improvements seen in the combination group suggest that bio-mechano-therapy may represent a meaningful step forward for refractory cases where time and tissue quality are critical.
