결과에서 피로 후 CAI군은 고관절 시상면 가동 범위가 줄고, 고관절과 슬관절에서 편심성 파워가 증가했다. 전체 충격 흡수에서 고관절과 슬관절의 기여 비율이 CAI군에서 더 높았고, 지면반력의 수직 성분도 CAI군에서 유의하게 증가했다. 즉, CAI 환자는 피로 시 발목 대신 무릎과 고관절로 충격을 흡수하려는 보상 전략을 사용하며, 결과적으로 지면반력이 증가해 재부상 위험이 커진다.
이 연구는 CAI 환자의 피로 상태 착지 전략 변화를 생체역학적으로 규명했으며, CAI 재활 시 피로 상태에서의 착지 훈련이 포함되어야 함을 시사한다.

Repeated ankle sprains can lead to chronic ankle instability (CAI). While CAI patients already exhibit landing difficulties, fatigue worsens the problem. Researchers from Yonsei University IOC Research Centre investigated the biomechanical mechanisms behind this phenomenon.
This study compared energy absorption strategies and ground reaction force during single-leg drop landing before and after fatigue in 20 CAI patients, 20 copers, and 20 healthy controls. The fatigue protocol involved repeated multidirectional running, hopping, and jumping until participants reached at least 85% of maximum heart rate and perceived exertion of 17 or above.
After fatigue, the CAI group showed reduced sagittal hip excursion, increased eccentric hip and knee power, and greater hip and knee contribution to total shock absorption. Vertical ground reaction force also increased significantly in the CAI group. Essentially, CAI patients under fatigue compensate by absorbing shock through the knee and hip rather than the ankle, resulting in increased ground reaction forces and elevated re-injury risk.
This study biomechanically characterized altered landing strategies in CAI patients under fatigue, indicating that rehabilitation should include fatigued-state landing training.
CAI rehabilitation requires more than simple ankle strengthening. Programs should incorporate balance and landing training under fatigued conditions, with emphasis on eccentric control at the ankle. Taping or bracing may serve as adjunctive protective measures during late-game fatigue.
