Can type 2 diabetes be put into remission through dietary intervention alone? Researchers at Tehran University of Medical Sciences compared calorie-carbohydrate restriction (CCR) alone with CCR plus intermittent fasting (time-restricted feeding, IFCCR).
One hundred twenty patients with type 2 diabetes were randomly assigned to CCR (n=40), IFCCR (n=40), or usual care (control, n=40) for a 6-month study consisting of a 12-week integrated lifestyle intervention phase followed by a 12-week maintenance and structured monitoring phase. Physical activity was emphasized in all groups.
At 6 months, diabetes remission (HbA1c below 6.5% without medication) occurred in 22.5% (9/40) of the CCR group, 30.0% (12/40) of the IFCCR group, and 2.5% (1/40) of controls. Both CCR (OR=11.7 vs. control, p=0.024) and IFCCR (OR=18.1 vs. control, p=0.007) significantly increased remission odds. However, the difference between IFCCR and CCR was not statistically significant (OR=1.5, 95% CI 0.6-4.3, p=0.4).
Both interventions significantly improved glycemic control. The key takeaway: calorie and carbohydrate restriction alone can achieve diabetes remission, and adding time-restricted feeding does not provide a clear additional benefit.
For diabetes patients, this means remission is possible without complex fasting protocols — simple calorie and carbohydrate reduction may suffice. While intermittent fasting may offer additional benefits for some, the current evidence does not show a significant difference, so choosing an approach that is sustainable for the individual matters most.
