어깨 회전근개 파열 수술을 앞둔 불안·우울 환자에게 항우울제 둘록세틴을 함께 처방하면 조기 회복 속도를 크게 높일 수 있다는 연구 결과가 나왔다.
서울대학교 분당병원 정형외과 연구팀이 주도한 이번 무작위 대조 임상시험에는 만성 회전근개 파열로 관절경 수술을 받은 63명의 환자가 참여했다. 연구팀은 병원 불안 우울 척도점수에 따라 참가자를 세 그룹으로 나눴다. 그룹 A는 심리적으로 건강한 환자, 그룹 B는 불안·우울 환자로 둘록세틴 복용군, 그룹 C는 불안·우울 환자로 위약 투여군이었다.
둘록세틴은 수술 당일 저녁부터 투여됐다. 처음 2주는 하루 30mg으로 시작해 이후 12주 동안 60mg으로 증량했다.
주요 결과인 수술 후 3개월 시점에서 그룹 B는 그룹 C보다 능동적 전방 굴곡 각도,기능 점수, 통증 점수,불안 점수 모든 지표에서 유의미하게 우수한 성적을 기록했다. 진통제 소비량도 그룹 B가 더 적었다. 특히 그룹 B와 심리적으로 건강한 그룹 A 사이에는 유의미한 차이가 없어, 둘록세틴이 심리적 취약 환자의 회복 수준을 정상 수준으로 끌어올릴 수 있음을 시사했다.
다만 그룹 B 환자의 16%(25명 중 4명)가 오심·구토로 인해 연구를 중도 포기했다. 제토제 투여로 증상은 완전히 해결됐지만 환자들은 스스로 투약을 중단했다. 수술 후 12개월 이후의 장기 기능 결과와 재파열률은 세 그룹 간 차이가 없었다.
회전근개 파열 수술 환자 중 상당수는 수술 전부터 불안·우울 증상을 안고 있으며, 이는 기존 연구에서 장기 예후 악화의 위험 요인으로 꼽혀왔다. 연구팀은 스크리닝 검사로 불안·우울 환자를 조기에 식별하고 둘록세틴을 처방하면 수술 후 3개월 내 회복의 질을 유의미하게 높일 수 있다고 결론지었다.

Patients undergoing arthroscopic rotator cuff repair who also suffer from anxiety or depression can achieve significantly faster early recovery when prescribed duloxetine, according to a new prospective randomized controlled trial.
Anxiety and depression are well-established risk factors for poor outcomes following orthopedic surgery. Yet targeted pharmacological strategies to address perioperative psychological distress in rotator cuff repair patients have remained understudied.
Researchers from Seoul National University Bundang Hospital enrolled 63 patients with chronic rotator cuff tears scheduled for arthroscopic rotator cuff repair and stratified them by Hospital Anxiety and Depression Scale scores into three groups: Group A (psychologically healthy, n=20), Group B (anxiety and/or depression receiving duloxetine, n=21), and Group C (anxiety and/or depression receiving no duloxetine, n=22). Duloxetine was administered orally starting the evening of surgery at 30 mg/day for the first 2 weeks, then escalated to 60 mg/day for 12 weeks.
At the 3-month postoperative assessment, Group B demonstrated significantly superior active forward flexion, functional outcome scores (ASES and SST), pain scores, anxiety scores, and reduced opioid consumption compared with Group C (all adjusted P < .05). Notably, outcomes in Group B were statistically indistinguishable from those in Group A, the psychologically healthy cohort, suggesting that duloxetine effectively offset the disadvantage conferred by psychological distress during early recovery.
However, 16% of Group B patients (4 of 25) withdrew from the study due to nausea and vomiting. Antiemetic treatment resolved symptoms completely in all cases, but patients elected to discontinue the medication. No severe adverse events were reported.
At later follow-up points of 6 and 12 months and beyond, functional outcomes and rotator cuff retear rates were comparable across all three groups, indicating that the benefits of duloxetine were concentrated in the critical early recovery window.
These findings carry practical implications for sports medicine and orthopedic surgeons. Systematic use of the HADS or similar tools to identify at-risk patients before shoulder surgery, followed by a short course of duloxetine, could meaningfully improve early rehabilitation outcomes and reduce reliance on opioid analgesia.
