유럽노인의학회 체계적 고찰 및 메타분석 특별 관심 그룹 연구팀은 PubMed, Scopus, Web of Science,코크란 도서관에서 성인 대상의 난드롤론 데카노에이트 대 위약 비교 무작위 대조 시험을 검색해 20편을 최종 분석 대상으로 선정했다. 연구는 2026년 4월 Journal of Cachexia, Sarcopenia and Muscle에 게재됐다.
주요 결과를 보면,난드롤론 데카노에이트는 제지방 연조직을 유의미하게 증가시켰다. 그러나 체지방량,악력,무릎 신전 근력에는 유의미한 효과가 없었다.
골밀도 결과는 일관되지 않았으며,대퇴 근위부 전체 골밀도만 일부 개선을 보였다. 출판 편향은 검출되지 않았으나, GRADE 평가에서 근거 확실성은 낮은 수준으로 분류됐다.

Nandrolone decanoate is one of the most studied anabolic agents in clinical medicine, used historically in conditions associated with muscle wasting and bone loss. Despite decades of use, a comprehensive quantitative synthesis of its effects on musculoskeletal health has been lacking. A new systematic review and meta-analysis published in the Journal of Cachexia, Sarcopenia and Muscle provides the most rigorous assessment to date.
Researchers from the European Geriatric Medicine Society's Special Interest Group in Systematic Reviews and Meta-analyses searched PubMed, Scopus, Web of Science, and the Cochrane Library through April 2025, ultimately including 20 randomized controlled trials comparing nandrolone decanoate to placebo in adults aged 18 and older. A random-effects meta-analysis was conducted with risk of bias assessment using the RoB2 tool.
Nandrolone decanoate significantly increased lean soft tissue by 1.59 kg (k=11; 95% CI 1.06-2.13, p<0.01, I squared = 0%) — a consistent finding across studies with no meaningful heterogeneity. However, the compound showed no significant effect on fat mass (SMD: -0.04, p=0.65), handgrip strength (SMD: 0.39, p=0.10), or knee extension strength (p=0.99).
Bone mineral density outcomes were inconsistent across sites and measurement approaches, with only total proximal femur BMD showing improvement (p<0.05). No publication bias was detected, though GRADE assessment rated the overall certainty of evidence as low.
The disconnect between measurable changes in lean tissue mass and the absence of functional strength improvements is clinically important. Increases in lean tissue measured by body composition methods do not necessarily translate into improved force-generating capacity or musculoskeletal function — a distinction often overlooked in anabolic therapy discussions.
The authors conclude that these findings do not support the routine clinical use of nandrolone decanoate for improving musculoskeletal health. Given its known safety concerns — including cardiovascular risk, hormonal disruption, and hepatic effects — non-pharmacological approaches such as resistance training and nutritional support should remain the preferred strategy.
