If you have ever experienced a sudden "pop" in your knee accompanied by excruciating pain during exercise, the diagnosis of a meniscus tear can feel overwhelming. In particular, complex tears—where the meniscus lifts like a bucket handle or is torn completely across—often leave patients hesitant to return to sports for fear of re-injury. However, recently published clinical results are providing a new foundation of hope for these patients.
The Journal of Experimental Orthopaedics (J Exp Orthop) published the results of a cohort study tracking the surgical prognosis of 55 patients who suffered from severe forms of meniscus tears. The research team closely monitored the progress of patients who underwent the "all-inside" repair method—a technique that sutures the cartilage using specialized instruments—over a minimum period of two years between 2019 and 2025.
The postoperative outcomes were more stable than expected. There were zero cases of re-tear or re-operation in the first year (0%), with cumulative failure rates of 6% in the second year, 8.7% in the third year, and 17.8% by the fourth year. This indicates that even four years after surgery, over 80% of patients maintained healthy sutured cartilage and continued their daily lives and sporting activities.
Subjective patient satisfaction reached a remarkable 97.9%. Notably, 87.2% of all patients successfully returned to the field, and nearly half of them (48.9%) recovered their performance levels to the same extent as before the injury. All subjective indicators, including knee function scores and the Visual Analogue Scale (VAS) for pain, showed dramatic improvement compared to pre-operative levels.
However, correlations were found showing that outcomes varied depending on the type of tear and whether concurrent surgeries were performed. Bucket-handle tears of the medial meniscus or degenerative tears showed a relatively higher risk of re-tear. Conversely, cases where anterior cruciate ligament (ACL) reconstruction was performed simultaneously tended to have lower failure rates. This suggests that securing knee stability through ligament reconstruction may create a more favorable environment for the cartilage to heal.
This study demonstrates that even complex tears, which are difficult to suture, can show excellent mid-term survivorship when the appropriate surgical method is applied. These findings reinforce the academic consensus that suture repair, which preserves as much of the original cartilage as possible, is more advantageous for long-term knee health and arthritis prevention than meniscectomy, which simply removes the torn portion. However, as this study analyzed statistical correlations between a specific surgical method and a patient group, it has limitations and should not be interpreted as a direct causal relationship that guarantees identical results for every patient.
For those considering meniscus repair after a knee injury, two practical points should be remembered. First, it is crucial to receive an early diagnosis rather than neglecting the injury, ensuring the optimal timing for surgery is not missed. The study indicated a positive correlation between receiving appropriate treatment during the acute injury phase and better overall prognosis compared to chronic tears.
Second, for at least six months to one year following surgery, patients should avoid abrupt changes in direction or jumping movements that place excessive strain on the knee. The high survivorship rate of over 80% is only possible when supported by systematic rehabilitation and a gradual return to exercise. The key to a successful return is consistently performing rehabilitation exercises to strengthen the muscles around the knee while recording daily pain levels and consulting with a primary physician.
