Have you ever felt a sharp, stinging pain just below the knee when landing on the court after leaping high for a powerful spike? For those who enjoy sports involving frequent jumping, such as volleyball or basketball, this knee pain is often worn like a badge of honor. In reality, however, it is a signal that the knee tendons are screaming for help. If left unattended, the tendons can degenerate, leading to a chronic condition known as "Jumper's Knee" (patellar tendinopathy).
To identify the key factors threatening the knee health of elite volleyball players, Portuguese researchers conducted an observational study involving 78 players from the Portuguese first division. Published in the Journal of Sports Medicine and Physical Fitness, the study utilized precision ultrasound imaging to examine the condition of the players' knee tendons and combined this with pain surveys to analyze the correlation between injury risk and training habits.
The analysis revealed that the most decisive variable determining knee health was training volume. Specifically, for every additional hour of training per week, the probability of developing patellar tendinopathy surged by 1.37 times, or 37%. This objective figure demonstrates that increasing exercise does not solely lead to improved performance; it can also threaten the recovery threshold that the knee tendons can withstand.
The gap in injury risk based on sex was even more striking. Male athletes were 4.41 times more likely to suffer from patellar tendinopathy than female athletes. This is interpreted as a result of male players generally exerting more explosive jumping power and muscular strength, which places a significantly higher load on the knee tendons during landing. In essence, the greater the power generated, the harsher the toll on the knees.
Position on the court also showed differences in risk levels. Attackers and setters tended to have patellar tendinopathy risks 6.18 times and 5.81 times higher, respectively, than liberos, who specialize in defense. Although these figures did not reach the threshold of statistical significance, they suggest that positions requiring more frequent jumping and explosive movements are more prone to knee tendon damage.
The significance of this study lies in providing concrete numerical evidence for the relationship between training volume and injury, which had previously been understood only vaguely. In particular, the fact that injury risk differs by more than fourfold depending on sex is a critical point that coaches must consider when designing training programs. However, because the study relied on a relatively small sample size of 78 participants, the lack of statistical certainty regarding position-specific risk remains a limitation.
To prevent knee injuries and extend an athlete's career, smart management is required rather than unconditional repetitive training. It is essential to avoid sudden increases in total weekly training hours and to immediately adjust training intensity the moment pain is detected. Specifically, it is recommended to correct landing postures after jumping to distribute the impact on the knee and to integrate eccentric exercises—exercises where the muscle generates force while lengthening—into training routines to strengthen the tendons.
