PubMed, Embase, Web of Science,Cochrane Library

Knee injuries — particularly anterior cruciate ligament (ACL) tears — are among the most devastating setbacks an athlete can face, often requiring months of rehabilitation and carrying a high reinjury risk. Can targeted prevention training meaningfully reduce these risks? A large-scale meta-analysis now provides compelling evidence that it can.
Published in *Frontiers in Public Health*, this systematic review and meta-analysis by researchers at Jiangsu Normal University in China analyzed 10 randomized controlled trials with a combined 15,296 participants. Studies were sourced from PubMed, Embase, Web of Science, and the Cochrane Library.
The results strongly supported the value of preventive training. Athletes in prevention training groups experienced a 32% lower incidence of overall knee injuries compared to controls (RR = 0.68, 95% CI: 0.51-0.92, P = 0.011). Even more striking, the incidence of ACL injuries was reduced by 57% in trained groups (RR = 0.43, 95% CI: 0.25-0.74, P = 0.002). Programs featured strength training, balance training, and plyometric exercises — all exercise-based injury prevention approaches.
Subgroup analyses were also performed, though the authors note these findings require cautious interpretation due to small sample sizes within subgroups. Overall heterogeneity was high, and the sources of this variability could not be fully identified — a common limitation in injury prevention research.
The review was prospectively registered with PROSPERO (CRD42024543884), lending additional methodological credibility.
Practical takeaway: Coaches and athletes should integrate knee injury prevention exercises into regular training routines. Effective programs typically combine hamstring and quadriceps strengthening, balance and proprioceptive training, and plyometric exercises. Youth athletes and those in high-contact sports stand to benefit most from consistent implementation.
