전방십자인대재건술 후 재활에서 하지 Y-밸런스 검사는 복귀 준비도를 평가하는 핵심 도구로 널리 사용된다. 수술 부위가 선수의 우세 다리인지 아닌지가 이 검사 결과에 영향을 미치는지에 대한 의문이 있었으나, 새로운 연구에서 그 차이가 유의미하지 않음이 확인됐다.
오클라호마대와 텍사스 여자대를 포함한 미국 공동 연구팀이 ACL 재건술 후 12주 시점의 청소년 운동선수 110명을 후향적으로 분석했다. 연구는 2026년 4월 International Journal of Sports Physical Therapy에 게재됐다.
수술 부위가 우세 다리인 선수 50명,비우세 다리인 선수 60명으로 구분해 YBT-LQ 수행 능력을 비교했다. 우세 다리는 공을 차는 쪽 다리로 정의됐다. 2 ×4혼합 분산분석이 수술 다리와 비수술 다리 각각에 대해 수행됐다.
결과적으로 수술 다리와 비수술 다리 모두에서 집단×방향 상호작용 효과나 우세 다리 주효과가 유의미하지 않았다. 효과 크기도 코헨의 d = 0.03~0.17 수준으로 미미했다.
이전 연구들에서 우세 다리가 비우세 다리에 비해 ACL 재부상률이 낮다는 보고가 있었고, 이로 인해 우세 다리 여부가 재활 예후와 복귀 시점을 결정하는 변수가 될 수 있다는 가설이 존재했다. 그러나 이번 연구는 적어도 수술 후 12주 시점의 YBT-LQ 결과에서는 이 요인이 의미 있는 차이를 만들지 않음을 시사한다.
다만 연구팀은 이번 결과가 12주라는 특정 시점에 한정된 것이므로, 복귀 시점이나 장기 예후와 우세 다리의 관계는 별도로 검토해야 한다고 강조했다.

The Y-Balance Test Lower Quarter (YBT-LQ) is a widely used tool in return-to-sport test batteries following anterior cruciate ligament reconstruction (ACLR). Prior research has suggested that dominant limb status is associated with lower rates of second ACL injury, raising the question of whether surgical limb dominance affects YBT-LQ performance during early rehabilitation — a factor that could inform prognosis and clinical decision-making.
A retrospective cohort study published in the International Journal of Sports Physical Therapy investigated this question in 110 young athletes assessed at 12 weeks post-ACLR. Participants were grouped by whether the reconstructed limb was their self-reported dominant limb (n=50) or non-dominant limb (n=60), with dominance defined as the limb used to kick a ball.
Separate two-way mixed ANOVAs (2 groups x 4 YBT-LQ directions) were conducted for both the surgical and non-surgical limbs. No significant group-by-direction interactions or main effects of surgical limb dominance were found for either the surgical limb (p=0.527) or the non-surgical limb (p=0.207). Effect sizes were trivial across all comparisons (Cohen's d = 0.03 to 0.17).
These findings suggest that limb dominance should not be treated as a primary determinant of YBT-LQ outcomes at the 12-week post-ACLR timepoint. Clinicians performing early post-operative assessments can interpret YBT-LQ results without adjusting expectations based on whether the surgical limb is dominant or non-dominant.
It is worth noting that this study was limited to the 12-week timepoint; the relationship between limb dominance and YBT-LQ performance at later rehabilitation stages or at return-to-sport clearance has not been evaluated by this research. Additionally, the study was retrospective and relied on a single clinic's database, which may limit generalizability.
