Many people notice that their strength is not what it used to be as they age. While the decline is often attributed to nerve function or hormonal changes, the root cause may be simpler: the loss of muscle mass itself. Researchers at the California Pacific Medical Center Research Institute tracked muscle mass and function changes across 895 adults ranging from their 30s to over 90.
The team used D3-creatine dilution — a method that measures creatine concentration in muscle to accurately calculate total body muscle mass — rather than conventional MRI. They assessed 69 adults aged 30-69 and 826 adults aged 70+, measuring muscle mass, thigh muscle volume, leg strength and power, walking speed, and cardiorespiratory fitness (VO2peak) simultaneously.
In men, muscle mass declined starting in the 30s, falling 0.5% per year during young adulthood and accelerating to 1.4% per year in the oldest-old. This decline pattern closely paralleled losses in leg strength and walking speed. In women, muscle mass was already decreasing at 0.6% per year in young adulthood but remained relatively stable thereafter, while strength, power, and cardiorespiratory fitness declined more steeply than muscle mass itself.
The key conclusion: muscle mass loss plays a larger role in functional decline than previously estimated. Preserving muscle mass starting in the 30s is critical for maintaining function in later life.
Practically, this means engaging in resistance training 2-3 times per week from your 30s onward, maintaining protein intake at 1.2-1.6g per kg of body weight, and incorporating aerobic exercise. Slowing the rate of decline early is more effective than trying to reverse it later.
