Fans' hearts sink whenever they see a player suddenly collapse on the pitch and be substituted. Almost everyone has experienced the anxiety of wondering whether it is a simple bruise or a catastrophic injury that signals the end of a season. In football, where intense physical clashes and full-speed sprints are constant, injuries are often viewed as an inevitable fate.
This study was published in the prestigious sports medicine journal, BMJ Open Sport & Exercise Medicine. It is a prospective cohort study involving 1,514 male and female players from the 1st and 2nd Bundesliga, Germany's professional football leagues. Researchers tracked injury and illness data for 963 men and 551 women through a national registry system to analyze health management characteristics by gender.
An analysis of 1,185 cases occurring over one season revealed distinct differences in the primary areas of injury between male and female players. Male players showed the highest incidence of thigh injuries, followed by the ankle and knee. In contrast, knee injuries were most frequent among female players, followed by the ankle and thigh. This suggests that while men experience greater loads on muscles responsible for acceleration and deceleration, women exhibit greater vulnerability in terms of joint stability.
Regardless of gender, the most common type of injury was identified as ankle sprains. These injuries, often described as "twisting an ankle," were the most frequent obstacles plaguing players during matches. However, in terms of time to return to play, a different injury proved most devastating. Anterior cruciate ligament (ACL) ruptures recorded the longest absence across all injury types, with female players tending to require more time for recovery and return to play than their male counterparts.
Trauma was not the only factor hindering players. The total number of days missed due to illness reached 3,188, with respiratory infections being the primary cause. This indicates that immune system management is as critical a factor in determining player availability as intense physical training. With 176 cases reported in men and 144 in women, the data proves that maintaining overall condition is vital for sustaining team strength regardless of gender.
This study is significant as it is the first in the history of professional German football to utilize national registry data to compare injury patterns between male and female players on a large scale. In particular, it clearly identified the injury characteristics of professional women's football—an area that has been relatively under-researched—highlighting the need for tailored management. However, as this is a cohort study observing a group, it should be noted that it demonstrates an association between specific genders and injury sites rather than establishing direct causation.
Amateur football enthusiasts can also apply these findings to their own routines. For men, focusing on eccentric contraction exercises—where the muscle generates force while lengthening—is effective for strengthening the thigh muscles (hamstrings). For women, combining balance and proprioceptive training to protect the knee joints is recommended. Furthermore, performing at least 15 minutes of dynamic stretching before exercise and managing immunity through adequate nutrition remain the most reliable ways to prevent both injury and illness.
