전방십자인대재건술 후 대퇴사두근 기능 회복을 정확히 예측하는 일은 재활 계획과 선수의 기대 관리에 핵심적이다. 위스콘신대 연구팀이 베이지안 확률론적 모델을 통해 개인 맞춤형 회복 예측을 현실화하는 방법을 제시했다.
연구팀은 디비전 I 대학 운동선수 66명이 ACL 재건술 후 1~14개월에 걸쳐 연속적으로 받은 등척성 대퇴사두근 검사 데이터를 분석했다. 측정 지표는 최대 토크하지 대칭 지수,토크 발달 속도 LSI,수술 다리의 토크 안정성이었다. 연구는 Sports Health에 2026년 4월 게재됐다.
다양한 복잡도의 베이지안 위계 B-스플라인 모델을 비교한 결과,이식편 종류와 술 후 시간을 반영하며 선수별 개인 무선 절편을 포함한 모델이 가장 정확한 예측을 보여줬다. 주목할 만한 것은 단 2회의 측정만으로도 높은 예측 정확도에 도달했다는 점이다.
임상 목표 달성에 걸리는 시간도 모델을 통해 산출됐다. PT는 평균 10개월, TS는 6개월, RTD는 12개월이 소요돼 RTD가 가장 오랜 회복 시간이 필요했다. 이는 RTD가 현재 복귀 기준에서 과소평가되고 있을 가능성을 시사한다.
이번 연구의 가장 큰 의의는 개념 증명 수준에서 임상적으로 현실적인 '개인화된 회복 궤적 예측 도구'의 가능성을 보여줬다는 점이다. 치료사는 이 모델을 활용해 개별 선수의 초기 측정 데이터만으로도 회복 경로를 예측하고 중재 타이밍을 최적화할 수 있다.

Accurate forecasting of quadriceps recovery after anterior cruciate ligament reconstruction (ACLR) has the potential to transform how clinicians set expectations, time interventions, and personalize rehabilitation programs. A new study from the University of Wisconsin-Madison demonstrates the feasibility of this approach using advanced Bayesian statistical modeling.
Researchers followed 66 Division I collegiate athletes (35 female; 317 total assessments) through serial isometric quadriceps performance testing at monthly intervals from 1 to 14 months post-ACLR. Outcomes included peak torque (PT) limb symmetry index, rate of torque development (RTD) limb symmetry index, and torque steadiness (TS) of the surgical limb. The study was published in Sports Health.
Multiple Bayesian hierarchical B-spline models of varying complexity were compared using a novel cross-validation design that mimicked real-world clinical use — iteratively increasing the number of observed assessments to test how prediction accuracy improved with more data. The best-performing model allowed recovery curve shapes to vary by graft type and included athlete-specific random intercepts.
Accurate predictions were achievable after as few as two assessments, with root mean square errors of 0.109, 0.173, and 0.536 for PT, RTD, and TS respectively. These errors are clinically acceptable for early-stage forecasting.
Estimated time to reach established clinical targets was 10 months for PT (90% symmetry), 6 months for TS (0.5% coefficient of variation), and 12 months for RTD (85% symmetry). The finding that RTD took the longest to meet clinical targets is particularly noteworthy — RTD reflects the neuromuscular capacity for rapid force production and may be underemphasized in standard return-to-sport criteria that focus primarily on peak torque.
The authors frame this as a proof-of-concept study with real clinical promise. As more data are incorporated through serial assessments, prediction accuracy improves — enabling more individualized and adaptive rehabilitation planning.
