하루 중 앉아 있거나 가볍게 움직이는 시간을 중고강도 신체활동으로 바꾸기만 해도 만성폐쇄성폐질환발생 위험을 크게 낮출 수 있다는 대규모 연구 결과가 나왔다.
베이징대 제1병원 및 베이징 유니언 의과대 공동 연구팀은 영국 바이오뱅크에서 유효한 손목 가속도계 데이터를 보유하고 기존 COPD가 없는 87,656명을 분석했다. 등시 대체 모델을 이용해 하나의 활동 시간을 다른 활동 시간으로 대체했을 때의 COPD 발생률 변화를 추정했다. 연구는 2026년 4월 Pulmonology에 게재됐다.
분석 결과,하루 1시간의 수면을 중고강도 신체활동으로 대체하면 COPD 위험이 40% 감소했다. 좌식 행동을 대체할 경우 43%, 저강도 신체활동을 대체할 경우 39%낮아지는 것으로 나타났다. 즉 어떤 활동 시간을 MVPA로 대체하든 실질적인 COPD 예방 효과가 있었다.
유전적 위험군별로 층화 분석한 결과에서도 이 역상관관계는 각 유전 위험군 내에서 유의미하게 유지됐다. 특히 중간 및 낮은 유전적 소인을 가진 그룹 중 MVPA 시간이 낮은 참여자는 유전적 소인이 높고 MVPA 시간이 많은 참여자에 비해 COPD 위험이 각각 1.72배,1.39배 높았다.
이번 연구는 MVPA가 유전적 COPD 위험을 완화할 수 있다는 점을 시사하며, 규칙적인 중고강도 신체활동이 폐 건강 보호에 있어 매우 중요한 수정 가능한 요인임을 확인시켜 준다. 하루 단 1시간의 활발한 신체활동 증가만으로도 상당한 예방 효과를 기대할 수 있다는 점에서, 운동 참여를 통한 공중 보건 개입의 잠재력이 크다.

Replacing sedentary time or light physical activity with just one hour per day of moderate-to-vigorous physical activity (MVPA) can reduce the risk of developing chronic obstructive pulmonary disease (COPD) by up to 43%, regardless of genetic predisposition — according to one of the largest studies to examine this relationship to date.
Researchers from Peking University First Hospital and Peking Union Medical College analyzed data from 87,656 UK Biobank participants who had valid wrist-worn accelerometer data and no prevalent COPD at baseline. Using isotemporal substitution modeling — a statistical approach that estimates the effect of reallocating time from one behavior to another while keeping total time constant — the team quantified the COPD risk reduction associated with replacing various movement behaviors with MVPA.
The findings were consistent: replacing one hour per day of sleep with MVPA was associated with a 40% lower risk of incident COPD, replacing sedentary behavior yielded a 43% reduction, and replacing light-intensity physical activity produced a 39% risk reduction. The direction and magnitude of these effects were remarkably similar across all three substitution scenarios, reinforcing the message that any increase in MVPA represents a meaningful step toward lung health protection.
Strikingly, the protective association held across all genetic risk subgroups. In joint analyses of genetic susceptibility and MVPA duration, participants with intermediate and low genetic predisposition who also had low MVPA were at significantly elevated COPD risk: 1.72-fold (95% CI: 1.32-2.24) and 1.39-fold (95% CI: 1.04-1.87) higher risk, respectively, compared to those with high genetic predisposition plus high MVPA.
The implication is clinically meaningful: MVPA can attenuate genetic COPD risk, making it a powerful and modifiable protective factor. These findings reinforce public health messaging around exercise as a lung disease prevention strategy, and suggest that even genetically susceptible individuals can substantially reduce their COPD risk through regular vigorous physical activity.
