발목 외측 인대 염좌 후 재활과 복귀 결정에서 발목 외번 근력은 핵심 평가 항목이다. 그러나 이를 측정하는 다양한 도구들의 신뢰도와 측정 오차가 얼마나 되는지는 명확히 정립되지 않았다. 벨기에 리에주대 연구팀의 체계적 고찰이 이 공백을 채웠다.
연구팀은 MEDLINE, Embase, Scopus, DiTA를 검색해 4,047건 중 23편의 연구를 최종 분석 대상으로 선정했다. 신뢰도는 급내 상관계수 또는 피어슨 r ≥ 0.70,측정 오차는 최소 감지 변화,일치 한계,변동계수 ≤ 20%를 충분 기준으로 정의했다. 연구는 2026년 4월 Journal of Sport Rehabilitation에 게재됐다.
포함된 도구는 등속성 동력계, 핸드헬드 동력계, 수직 도르래, 맞춤 장치였다. 참여자의 90%는 건강한 대상자, 10%는 발목 외측 인대 염좌 환자였다.
전체 측정값의 83%에서 신뢰도가 충분 기준을 충족했으나, 측정 오차는 34%에서만 기준을 만족했다. 이 차이는 임상 및 연구 현장에서 신뢰도는 수용 가능해도 측정 오차가 문제가 될 수 있음을 시사한다.
등속성 동력계, 특히 등척성 검사에서 가장 일관된 신뢰도와 낮은 측정 오차가 확인됐다. 등속성 장치 이용이 어려운 경우 핸드헬드 동력계가 등척성 검사에서 신뢰할 수 있는 현장 대안이 될 수 있다. 동적 근력 평가에는 1회 최대 반복 방법이 사용될 수 있으나 측정 오차가 더 크다.
무릎 위치, 안정화 방법, 시각적 피드백, 언어적 격려 등의 프로토콜 매개변수가 측정 오차와 평가자 간 신뢰도에 영향을 미치는 것으로 나타났다. 따라서 재현성 향상을 위해 측정 프로토콜의 표준화가 필수적이다.

Ankle evertor strength assessment is a cornerstone of rehabilitation decision-making following lateral ankle sprain, yet the reliability and measurement error of the instruments used for this purpose have remained poorly synthesized. A systematic review from the University of Liège provides the most comprehensive analysis of this topic to date.
Researchers searched MEDLINE, Embase, Scopus, and DiTA without date restrictions, ultimately including 23 studies from an initial pool of 4,047 records. Sufficient reliability was defined as intraclass correlation coefficient or Pearson r greater than or equal to 0.70, and sufficient measurement error was defined as minimal detectable change, limit of agreement, or coefficient of variation at or below 20%. The study was published in the Journal of Sport Rehabilitation.
Instruments reviewed included isokinetic dynamometers, hand-held dynamometers, vertical pulleys, and custom instruments. The participant sample was predominantly healthy individuals (90%), with 10% having a history of lateral ankle sprain. Methodological quality was rated adequate in 35% of studies, doubtful in 39%, and inadequate in 26%.
Reliability was sufficient in 83% of extracted measurement values, but measurement error met criteria in only 34% — a notable gap that suggests many currently used measurement approaches produce acceptable consistency but unacceptably variable absolute error. This distinction matters clinically: a test can appear reliable while still yielding errors too large to detect meaningful change in individual patients.
Isokinetic dynamometry, especially in isometric testing, demonstrated the most consistent reliability and lowest measurement error across included studies. When isokinetic equipment is unavailable, hand-held dynamometers offer a reliable field alternative for isometric testing. One-repetition maximum methods may be used for dynamic strength evaluation, though with higher measurement error.
Protocol parameters significantly influenced outcomes. Contraction velocity, knee position, and verbal encouragement affected measurement error, while knee position and stabilization methods influenced interrater reliability. The authors call for standardized protocols to improve measurement reproducibility across clinical and research contexts.
