전방십자인대 재건술 후 지속적인 무릎 증상을 겪는 청년 환자 중 평균 2.3년이 지나도 부상 전 종목으로 복귀한 비율이 18%에 불과한 것으로 나타났다.
호주 라트로브 대학교 스포츠·운동의학 연구센터가 "Journal of Sport Rehabilitation"에 발표한 이번 연구는 SUPER-Knee 임상시험 데이터를 활용해 ACL 재건술 후 9~36개월 사이에 지속적인 무릎 증상을 호소하는 18~40세 환자 184명을 분석했다.
분석 결과, 부상 전 스포츠를 즐기던 참가자 중 부상 전 종목으로 복귀한 비율은 18%(28명), 어떤 스포츠든 복귀한 비율은 24%(37명)에 그쳤다. 반면 주당 신체활동 횟수 중앙값은 부상 전과 수술 후 9~36개월 시점 간에 큰 차이가 없었다. 이는 환자들이 스포츠 복귀에는 어려움을 겪으면서도 걷기·수영·자전거 타기 등의 일반적 신체활동은 유지하고 있음을 보여준다.
스포츠 복귀와 유의미하게 연관된 인자는 단일 전진 점프 하지 대칭성 90%이상 달성 여부였다. 이 기준을 충족한 참가자는 부상 전 종목 복귀 오즈비 3.95,어떤 스포츠든 복귀 오즈비 2.97로 유의미하게 높은 복귀 가능성을 보였다. 나이, 성별, 수술까지의 기간, 민간 의료보험 여부, 대퇴사두근 근력 등 다른 변수는 유의미한 연관성을 보이지 않았다.
연구팀은 지속적 증상 환자군에서의 낮은 스포츠 복귀율을 고려할 때, 기능적 점프 대칭성 달성을 재활의 핵심 목표로 설정할 필요가 있다고 강조했다.
Among young adults experiencing ongoing knee symptoms following anterior cruciate ligament reconstruction (ACLR), only 18% had returned to their preinjury sport by an average of 2.3 years post-surgery, according to a new study from the SUPER-Knee Trial published in the Journal of Sport Rehabilitation.
Researchers from La Trobe University Sport and Exercise Medicine Research Centre analyzed 184 participants aged 18 to 40 years who reported unacceptable ongoing symptoms—including poor function, pain, and instability—between 9 and 36 months after ACLR.
The results revealed that of those who had played sport in the year before their ACL injury, only 18% (n=28) had returned to preinjury sport, and 24% (n=37) had returned to any sport. Despite these low sport return rates, the median number of weekly physical activity sessions was similar between the preinjury period (4 [IQR 2–6]) and at 9 to 36 months post-ACLR (4 [IQR 1–6]), suggesting patients were maintaining general physical activity such as walking, swimming, and cycling, even if sport-specific participation remained limited.
The only variable significantly associated with return to sport was achieving a single-forward hop limb symmetry index of ≥90%. Participants meeting this threshold had an odds ratio of 3.95 (95% CI 1.61–9.65) for returning to preinjury sport and 2.97 (95% CI 1.38–6.41) for returning to any sport. Age, sex, time between injury and surgery, private healthcare, and quadriceps strength were not significantly associated with return to sport outcomes.
The findings highlight the importance of targeting functional hop limb symmetry as a key rehabilitation goal to improve sport return rates in this challenging patient population.
