야구 선수의 팔꿈치 내측 척골 측부 인대 수술 건수가 계속 늘고 있는 가운데, 이 부상이 팔꿈치에 국한되지 않고 어깨 근력 전반에도 영향을 미친다는 연구 결과가 나왔다.
미국 피츠버그대 물리치료학과 레이놀즈 연구팀은 MUCL 부상 대학 야구 선수 35명과 포지션·투구 팔·경력 등을 맞춘 부상 없는 선수 35명, 총 70명을 비교 분석했다. 연구는 2026년 4월 Journal of Athletic Training에 게재됐다.
연구팀은 체중 대비 정규화한 등척성 스캡션, 외회전, 내회전 근력을 양쪽 팔 모두 측정했다. 스캡션이란 어깨를 앞쪽 45도 방향으로 들어 올리는 동작으로, 회전근개의 기능을 평가하는 데 사용된다.
분석 결과, 부상 없는 선수는 투구 팔 기준으로 스캡션 0.30 N/kg, 외회전 0.34 N/kg, 내회전 0.68 N/kg 더 강했으며, 비투구 팔에서도 각각 0.36 N/kg, 0.32 N/kg,0.58 N/kg의 차이가 확인됐다. 체질량지수와 경기 수준을 통제한 후에도 이 차이는 유의미하게 유지됐다.
특히 내회전 근력의 차이가 가장 컸다는 점이 주목할 만하다. 내회전 근력은 투구 시 가속 구간에서 핵심적으로 작용하며, 결핍이 있을 경우 팔꿈치 내측 구조물에 과도한 부하를 가중시킬 수 있다.
연구팀은 MUCL 부상 선수의 복귀 전 검사나 부상 예방 선별검사에 체중 보정 어깨 근력 지표를 반드시 포함해야 한다고 강조했다. 단순히 팔꿈치 기능 회복 여부만 평가해서는 안 되며, 어깨의 스캡션 및 회전 근력 상태를 함께 확인해야 한다는 것이다.

Baseball pitchers continue to see rising rates of medial ulnar collateral ligament (MUCL) injuries requiring surgery, yet rehabilitation programs have largely focused on elbow function in isolation. New research suggests the problem extends well beyond the elbow joint.
A matched cross-sectional study published in the Journal of Athletic Training examined 70 collegiate baseball players — 35 with MUCL injuries and 35 uninjured peers matched for position, throwing arm, years of participation, year in school, and age. Researchers measured body mass-normalized isometric scaption, external rotation, and internal rotation strength bilaterally.
The results were striking. Uninjured players demonstrated significantly greater strength than their MUCL-injured counterparts across all measures and both arms. On the throwing arm, the differences were 0.30 N/kg for scaption, 0.34 N/kg for external rotation, and 0.68 N/kg for internal rotation. On the non-throwing arm, differences of 0.36 N/kg, 0.32 N/kg, and 0.58 N/kg were observed respectively, even after controlling for body mass index and level of competition.
The largest deficit was in internal rotation strength, which plays a critical role during the acceleration phase of throwing. Weakness in this plane could contribute to increased valgus stress on the medial elbow structures — potentially a factor in both injury occurrence and recovery.
The bilateral nature of these deficits — present in both throwing and non-throwing arms — raises important questions about the directionality of the relationship. Do pre-existing shoulder strength deficits contribute to MUCL injury risk, or does the injury and subsequent disuse lead to overall upper extremity weakness?
For clinicians, the practical takeaway is clear: body mass-normalized scaption, external rotation, and internal rotation strength should be incorporated into return-to-sport screening for MUCL-injured athletes and may serve as valuable screening tools in uninjured pitchers to identify those at elevated injury risk.
