Whether it is a sudden "pop" heard during a soccer match or a moment of losing balance while lifting a heavy object that puts excessive strain on the knee, certain incidents can lead to severe injury. If this is accompanied by intense pain and an inability to fully extend the leg, there is a high probability of a patellar tendon rupture—the tendon that connects the kneecap to the shinbone. This injury, which can make even basic walking impossible, often leaves patients facing both a fear of surgery and anxiety over a long recovery period.
A recent cohort study published in the Spanish journal of Orthopaedics and Traumatology (Rev Esp Cir Ortop Traumatol) examined 46 adult patients with acute patellar tendon ruptures. The research team conducted a comparative analysis of the outcomes between two surgical methods for reconnecting the ruptured tendon: the traditional "transosseous tunnel" technique, which involves drilling holes through the bone to pass sutures, and the "suture anchor" technique, which uses small screws to secure the tendon to the bone.
The analysis revealed that surgery using suture anchors reduced the average operative time by approximately 20 minutes compared to the traditional method. Specifically, the average surgery time for the suture anchor group was 78.7 minutes, whereas the transosseous tunnel group averaged 99.6 minutes. This improvement in efficiency is attributed to the direct insertion of anchors, eliminating the complex process of drilling tunnels.
Despite the shorter operative time, there was no difference in the quality of treatment. No statistically significant differences were found between the two groups regarding postoperative pain indices, knee range of motion, or the rate of return to daily activities and sports. Patient-reported knee function scores and quality-of-life indicators also showed equally excellent results for both methods. Furthermore, the rates of reoperation and postoperative complications were similar.
This study demonstrates that the suture anchor method effectively shortens surgical time while providing a level of recovery comparable to the current standard surgical procedure. However, it is important to note that due to the nature of cohort analysis, this study does not establish a definitive causal link proving one method is inherently superior, but rather shows a correlation between the surgical method and reduced operative time. Additionally, there are limitations in that detailed intraoperative variables were not perfectly controlled.
For those facing surgery for a patellar tendon rupture, it is advisable to ask their specialist specifically about the surgical method to be used and the expected duration of the procedure. A shorter surgery time can be advantageous as it reduces the duration of anesthesia, thereby decreasing the overall physical burden on the body.
Regardless of the surgical method, consistent rehabilitation is the key to recovery. During the first six months following surgery, patients should perform gradual quadriceps strengthening exercises under professional guidance. In particular, performing range-of-motion exercises for knee flexion and extension regularly—at least three times a day—is the most reliable way to ensure a successful return to activity.
