Youth tennis participation has been growing steadily, but understanding how injuries differ between boys and girls has remained limited. Researchers at Rutgers New Jersey Medical School analyzed 10 years of emergency department data from 2014 to 2023 to shed light on this question.
The analysis covered 1,679 pediatric tennis-related injuries, corresponding to an estimated 48,368 cases nationally. Males accounted for 53.1% and females 46.9% of injuries, with males peaking at age 14 and females at age 16.
The most striking differences were in injury type and location. Males experienced significantly more fractures (p<0.001) and lacerations (p<0.001), with injuries concentrated in the upper extremities — particularly the elbow — as well as the eyeball and face. Females, on the other hand, had significantly more ankle injuries (p<0.001) and a higher prevalence of strains and sprains (p<0.001). By age, males sustained more injuries during elementary school years, while females had more injuries during high school.
These differences likely stem from biomechanical and neuromuscular factors: females tend to land with knees collapsing inward, while males engage in higher-impact play styles. The high rate of facial and ocular trauma in males supports the need for protective equipment.
For practical application, young tennis players should receive sex-specific neuromuscular training — landing posture correction and strength balance exercises. Male players may benefit from facial protection and elbow guards, while female players should prioritize ankle stabilization exercises and lower limb alignment correction.
